PCOS and testosterone - advice and experiences needed

Advice Needed

Hi I am a trans man 10+ years on t posting on behalf of my partner 2 years on t.

My boyfriend and I suspect he has PCOS but he never went to get it checked properly before starting t.

Reasons we suspect are:

  • he has had massively irregular periods for years before taking t, sometimes bleeding for almost a month, sometimes no period for up to 12 weeks.
  • pre testosterone labs indicated slightly higher testosterone than normal for a cis f
  • he got the progesterone coil before taking t to help with menstrual symptoms. It worked, but he still had breakthrough bleeding and emotional symptoms
  • he has frequent non diabetic hypoglycaemic episodes (I am type one and can confirm this by finger pricking him)
  • on testosterone, he has breakthrough bleeding even when t levels are in typical male range, causing me to suspect he aromatises at a high level

We live in the uk and have brought these symptoms to his endo and gp who seem weirdly unconcerned.

The reasons we would like for him to get checked out are:

  • it may impact what administration route and protocol he uses to take t, in order to ensure levels are in range
  • he may need his e2 to be monitored more closely to prevent breakthrough bleeding and emotional symptoms
  • we would like to lay the groundwork for pursuing a hysto if his symptoms continue to be difficult to manage
  • there may be various thyroid and diabetes- related conditions that he should be aware of if he does have PCOS

His endo and gp keep wondering why he would care whether he has PCOS or not, now that he’s on testosterone. Am I the only one who finds that weird? It’s totally possible ofc that he doesn’t have PCOS but I’d like him to be able to get imagine done if possible as the symptoms are pretty distressing for him.

Any advice from other people who’ve been through similar would really help, esp if based in the uk.

Thanks in advance.
