Matthew 15:
19 For out of the heart come evil thoughts—murder, adultery, sexual immorality, theft, false testimony, slander.

Does porneia mean primarily prostitution, or can it mean sex between people not married?
BDAG πορνεία:
① unlawful sexual intercourse, prostitution, unchastity, fornication
Those were the primary meanings.
Oxford dictionary fornication:
the act of having sex with somebody that you are not married to
Is sex outside of marriage a sin?
Yes, it is sexual immorality (porneia).
How is responsible sex before marriage morally wrong?
I'm not sure if it is morally wrong. It is not a crime in Canada. However, Jesus said porneia was a sin.
But responsible sex does not hurt anyone?
God decides what sin is, not me. Eve ate the forbidden fruit. She acquired the ability to determine what was good or bad (sin) independently of God. Now, we all have this ability. I choose not to exercise this particular ability, but I depend on God to tell me what is sin and what is not.
