Mental leave?

Has anybody taken a mental leave I’m a TL for deli/bakery. I just came back from being off and being thrown stuff back and forth. If you’ve taken a mental leave, what are the steps or documents required to take one? No I don’t want to ask my PL she’s always damn busy.’
