Referral vetting issue

Medical Politics

Got a CT head request for a 6‑year‑old from an ACP in ED. I asked if a consultant or reg had reviewed, and she said the consultant isn’t on site and that she “works at registrar level” so doesn’t need to get things checked by another doctor.

It’s a paediatric head CT with ionising radiation and I’m being asked to justify it under IR(ME)R with no doctor oversight. I’m not trying to block the scan or delay care, but I’m really uneasy just accepting this on the basis of an ACP’s self‑declared equivalency.

How best to handle this? • I want to stay professional and keep the patient pathway moving. • But I also want to push back clearly on the claimed equivalency being mindful of governance and safety grounds. • Do trusts actually allow ACPs to request paeds CTs without a doctor reviewing first?

Looking for advice on how to respond and set boundaries without coming across as obstructive.
