Ambiguous coding in medical record for seizures? (27F)

Hi AskDocs,

I (27F) have a history of focal seizures and have been on an antiepileptic medication for many years. I'm always so confused when I look at the ICD codes in my medical records!

Sometimes I'll go to the doctor and they'll code under G40 while other times they'll code under R56. I've had an abnormal EEG showing epileptiform spikes in a specific region of my brain. Even when my EEGs are normal, they typically show nonspecific findings that are, from what I gather reading peer-reviewed literature, common in folks with focal epileptic seizures.

FWIW, I was also diagnosed with autism as a kid and again as an older teenager, and have neither a personality nor conversion disorder. (Although unfortunately sometimes I think folks make assumptions about me given my gender and communication differences due to the autism + very high IQ combination (<not saying this last point to be arrogant but rather because I've noticed folks perceive me as different unless I'm in a room with fellow eggheads).)

Does anyone know why there's flip flop between the two ICD codes in my medical records? Is it just the product of being an in-between case where my history and abnormal EEG suggests focal epilepsy but they can't definitely say that's what it is, inconsistent knowledge between healthcare providers, and/or something else?
