Are Achievements broken on this game?

Playing on Xbox.....I've exfilled from Buried City, Dam, and Spaceport, which is the Well travelled achievement, and still didn't get it. On top of that, there's an Achievement for setting off 2 car alarms in 1 round, which I did....but it shows it at 99% complete. How is that possible?
