Patient comes in injured. While examining him, doctors hear him calling out “Margo”. When asked who Margo is, the patient named Joey Berger says Margo is his sister.

Amelia and Derek are Joey’s doctors and they tell him his family has been contacted so he’ll get to see his parents and sister. Joey says his sister is already with him and when Derek asks where Margo is, Joey points to the side of his bed, but Derek and Amelia don’t see anything. Joey says his sister is invisible.

Joey explains that he and Margo are twins and when they were born, they were born together, looking like a strange mutant thing with extra limbs and eyes. He had surgeries done and had scars but his sister’s brain is still inside of him, connected to his brain. Margo to him is a very real sister he loves to talk to and do fun stuff with. Joey “sees” Margo via projection, she sends him signals, telling his brain what she says and what she looks like.

Hearing this, both doctors treat this differently, Derek is respectful and answers questions Margo has, Amelia treats it like Joey has an anomaly and not a sister. While looking at his scans, they see the two brains in Joey’s head, two brains that are connected and was mistaken as one before. Joey also has a big problem in his chest they need to fix, Amelia says he could die if they don’t. Derek says it’s not just one life at stake but two, Amelia can’t believe Derek is seeing this extra brain as a person, Derek argues that is still a life, an individual life that is intelligent and full of love and if Amelia can’t see that then she shouldn’t do the surgery with him.

Joey’s parents arrive and Amelia continues to not respect Margo, telling the parents they shouldn’t be encouraging this delusion. The parents are aware the situation is strange but they love their daughter even if she’s a brain, Joey shares almost everything she says. Derek wants Amelia off the surgery but later during Derek lets her help, Amelia really wants to save both lives.

The surgery is a success and both Joey and Margo are good. Derek talks to the parents outside Joey’s room and Amelia finally talks to Joey and Margo, now calling her Joey’s sister and she enjoys the conversation they all have.
