What did I sign

My wife is in the clerk craft working at the plant. She called out sick two days last week and today they told her she had to sign some kind of paperowkr as a result. She has some disabilities so she wasn't able to tell me what the paperwork was about and they didnt give her a copy. She said shes had to do that in the past too. Her union rep was not present.

I know thats not much info but can anyone explain why and what she signed?
