I have a few quick questions - hope you can help!

取用中文姓名聲明書
For #1–8, should I use the guardian’s or child’s info? If guardian, is it the foreign parent?
At the bottom, do I put the child’s first and last name?

For #1–7 and #10, whose info goes in, the child’s or guardian’s?
In the red dotted box under "Please fill in following information, if applied by an agent," should I enter my info as guardian?
