Denied but finally received DBQ and am more confused than ever???
Good morning beautiful people! My claim for IBS was denied several week before the shutdown. I requested my DBQ and just received it today. I've attached a few snaps below.
Here are my questions. In section iii (3) BOTH boxes (a&b) were checked.....is that possible?? Also in Section 3C there are multiple statements of "Nexus Established". Am I missing something here or should I file an HLR?? As always THANK YOU for all your help and guidance. It is GREATLY appreciated!!
