K2 Kharshi/ Khanabad

Has anyone received disability from being deployed to K2? I have been requested to fill out some type of an exposure form and it is not anything that relates to what I have experienced since being deployed to K2. If anyone has would you mind sharing what type of documentation you used. TIA
