Pain and Bleeding during sex?

Hi all. Pretty much what the title says! I (24F) have been with my partner (27M) for about 6 months now, and almost everytime we have sex I have light bleeding and some pain. It feels like a friction issue, but I am almost certain there is enough "lubrication;" we have talked about picking up some lube to see if it helps. It isn't just him, as this has also been the case with previous sexual partners.
I am looking to see if anyone else has or has had this issue, and what remedies helped!! Thanks in advance.
