Did Thaddeus get a Ghoul serum, or was it FEV?

I ask this because I don't remember any Ghoul having a second mouth on their neck. The way he has mutated does look like someone who's flesh is rotting. We've seen the disgusting ways FEV can mutate a subject in the Master, Centaurs and Snallygasters. This seems in line with that.
