Visit Pakistan with baby

Pakistani lady with British husband and 1 year old British kid want to visit Pakistan. The husband got a visa quickly. Now when they apply for the kids visa, it has been rejected because 'missing consent form'. They cannot find anywhere to add this form. Anyone got any idea about this?
