Intermittent Bolt Lock On Empty Issue

Was at the range today and noticed some intermittent issues with bolt hold open on empty magazine. Happened maybe half the time for first 20-30ish rounds (about 5 or 6 diff mag loads at small capacities). Went away eventually.

Wondering what the issue might be? Low round count build (200ish maybe). KAK 16” mid length upper, aero lower. I do have both an ar maglock/kingpin and bad lever installed so potentially some problem there?

Had also just done a clean and lube of the bcg before hitting range. Part of me thinks maybe I was just accidentally touching bad lever. Any way to tell if the issue might be more of a BCG/gas system problem? It did consistently lock back after that first batch of mag loads though.
