What do you do when you've got ongoing medical referrals but you have to move?

Long story short, we've been forced to move. Our new home is half an hour away, well out of our current GP catchment. Driving times to the hospital I've been referred to for various things is the same, but we are closer to other hospitals now.

With a couple of diagnoses/referrals in progress... how on earth do I navigate the GP situation/bureaucracy of it all? Can I retain my current referrals after changing GPs or do I have to start them over?
