Help! Loach got hurt

So one of my kuhli loaches got her tail stuck in some moss and I didn't notice until she had clearly been trying to get out all night and had tired herself out. I cut her out and she's alive but recovering slowly. My real question isn't how to help her since I have her in my recovery tank with medicine but how to stop this from happening in the future. I have java moss the loaches love to hide in, but this has me worried I should try to rip it out and replace it with something safer. Was this just a freak accident or something that happens more commonly with java moss?
