Work restriction advice

I was given a doctors note for work restrictions as an RCA for 5/50 due to mental health / stress. What is the process for putting this restriction into effect? I don’t necessarily trust my management and would prefer to have them have as little to do with the process as possible.

Do I still give it to them first? Or go through district nurse? My steward really didn’t have much of an answer.
