Etiquette for signed feedback form

Just confirming professors themselves can see student names attached to feedback forms if their submitted with accepting the signing question?

What incentive does this give to students if the professor has a chance to teach this student for future course offerings? I understand that the signed comments are also forwarded to the dept head, but any negative comments have no incentive if anonymity is absent.
