Confused, tired, looking for some advice/past experiences-CT/biopsy results
Hey everyone,
I'm wondering what people's experiences are, especially re: to a very early crohn's disease diagnosis. I apologize if this is a long message, I've got quite a long and weird history I guess? Idk! Maybe I'm wrong lol.
Basically, I began having weird bowel movements last November 24' (blood in stool, erratic bowel movements (from constipated to going 6+ times a day), greasy stools, etc.). I also began experiencing chronic fatigue, a loss of appetite, and abdominal pain. In December I was diagnosed with pretty low iron. In January, I got a severely low vitamin D diagnosis. Come February, I began to lose significant weight. From the beginning of February to Feb 13th I went from 145 to 137lbs, and then continued to lose weight until end of March/beginning of April (went down to 130lbs). My chronic fatigue got worse and I began getting lightheaded/dizzy all the time, to the point of almost passing out on a daily basis. Loss of appetite, feeling easily full, etc.
I eventually had a endoscopy and colonoscopy done at the end of April, given my symptoms as well as my GP being concerned as my sister was just diagnosed in Dec with UC. My stomach scope was fine but my colonoscopy found redness at the TI (terminal ileum). My biopsy report came back with "focal acute ileitis NYD. Mild, patchy acute inflammation in the superficial lamina propria and apthous ulceration on the surface". I don't have any chronic changes and got a differential diagnosis of "acute/infectious ileitis, bowel preparation artifact, drug-related mucosal injusty, or very early Crohn's disease".
During my follow-up in October, my GI was intrigued with the very early crohn's disease mention on my biopsy, especially since I don't take NSAIDs, most likely isn't bowel prep, as well as the symptoms I was having (I was still experiencing lightheaded, loss of appetite, dizziness, low iron, erratic bowel movements, and some weight loss on and off - I weighed 125lbs). I have been seeing my GI on and off since 2017, cause I've had a weird history of GI issues that he's never been able to figure out. From 2017-2019, I dealt with two ulcers and gastritis in my stomach that took multiple treatments to heal and no definitive answer as to why those issues happened. In 2021, I had severe URQ pain and abnormal bowel movements (frequent, loose, fatty, greasy stools, at times orangey/sand colour). Had a endoscopy and MRI that came back normal, then a HIDA scan cause they thought it was my gallbladder but everything was normal. My GI had no idea what was wrong.
Anyways! He ordered me a CT enterography with IV contrast (what a horrible experience!) and I just got my results and am a bit confused. The results say: "There is very subtle inner lining enhancement involving the terminal ileum and the distal ileum. This segment measures approximately 10 cm. There is no perienteric stranding. No perienteric lymph node enlargement. No sinus tract or fistula. No skip lesions within the remainder of the small bowel. No stricture formation. The colon is within normal limits. No signs of colitis". The impressions at the end reads: "Inner lining enhancement involving the distal 10 cm of the ileum inclusive of the terminal ileum. No perienteric stranding. No skip lesions. No additional abnormality of significance. Clinical correlation will be most useful to assess the significance of this radiological finding".
So yeah, all in all, just wondering what people think (based on their own experience), especially in relation to a potential very early crohn's diagnosis. Is it possible to have very early crohn's without wall thickening? Or am I potentially looking at something else for whatever the hell is going on with my body lol. I know I have to wait until my GI appt in January to actually interpret these results, but yeah, any guidance, advice, or personal experience would be very much appreciated it!! I am quite tired and frankly so sick of being thrown back and forth between this test and that test - I live in Canada, so alas, everything takes forever here. I literally just want an answer and solution to all these symptoms (sigh).
