Did my dentist screw up my implant?

question

I had #18 removed and replaced with an implant in November of 2023 (I had those wisdom teeth removed years ago). I have since needed to come in to have the implant tightened twice, and now it has popped off completely.

The first time, I shrugged off - figured it was a fluke thing.

The second time as on October 30, 2025, and they suggested it was because I had stopped wearing my night guard. There were supposed to have ordered a specific screw they needed, but "it didn't arrive in time," so they tightened the one that was in place.

After the second tightening, I began wearing my night guard religiously every night, but it became loose again. I called in an appointment, and I'll be seeing them tomorrow. However, tonight the implant has completely come out while I was eating dinner. Photos linked below:

After that last tightening, I could feel the spot where the screw was, and it looks like maybe the resin didn't cure correctly, or came loose or something?

My question is - are my dentists just incompetent? I was told repeatedly that a dental implant is basically a one-time, fire-and-forget procedure and that taking care of it was no more hassle than taking care of my biological teeth. I am very frustrated and would like some perspective.
