Visa G1-10 : application website

Good morning everybody !

I am coming to you as it is a bit blurry how to apply for a G1-10 visa.

I have had a surgery in Korea under a normal 90 days tourist visa, which the hospital confirmed I need to stay for a longer period as I haven’t fully recovered yet. I have an official document from the hospital.

In order to apply for g1-10, can you confirm me this is properly where to apply for it ?
