Question about removal

Hi, I’ve had my 🍒 pierced for about 3 years and they still get crusty and often hurt. I’ve debated taking them out and with a surgery coming up feel it’s the best option. Best way to make sure they heal properly if I take them out? If I choose to keep them what can I do to make these issues stop?
