When to use the Test issue type in Jira

I am currently using Zephyr in Jira for test case management. I’ve been on a project where once we get a bug ticket or story ticket moved to the QA column in Jira, we would create a test case in Zephyr, attach it to the bug/story ticket, and then leave a comment with the test results. On a different project, for each story ticket we would create a separate Test ticket where we would attach our test cases to and add a comment with our test results. We would then link the test ticket to the story ticket. Which process is the most common? Is this how Test tickets should be used? Is there a better process?
