Darkest days of my life, feeling hopeless. Torn sesamoid ligament, healed, but now intense pain

In September, I tore my sesamoid ligament from climbing. I didn't get a proper diagnosis until 5 weeks later when I got an MRI. It showed a high grade tear of the sesamoid ligament. I was put in a boot for 6 weeks, and the pain subsided while in the boot. My podiatrist said I can walk without the boot. The second MRI says the tear has healed but now it shows I have edema in both sesamoids. The pain is worse than ever.

I've been to PT 3 times and when I explained that the pain is getting worse, she said I might need to go into the boot again, and that sesamoid pain is chronic pain that can only be managed through lifestyle changes.

The thing is, I have very poor mental health. I'm a congenital heart defect patient and have had 2 heart surgeries. After my second heart surgery at 25, I felt fantastic and my mental health improved because I was able to have an active lifestyle - I became a climber, and actively hiked, walked, played sports. I got a job as a sports photographer, which is an active mobile job. It was like a new lease on life and for the first time I was looking forward to my 30s.

Now I'm about to turn 31 and feel absolutely hopeless. All the anguish I've been through and I now will have to live the risky life I have with chronic pain and immobile.

Please share a story with me, will I ever get back to climbing and hiking? The PT hurts more than anything, and I'm feeling at my end. Can this heal?
