Do 4 premolar extractions really make sense in my case? Would love some opinions.

question

Hey everyone,

I’m 22 and I just had an orthodontic evaluation that left me a bit unsure, so I wanted to ask here before making a big decision.

My teeth are already straight from previous braces, and when I look at my bite it honestly seems Class I — molars and canines line up well. The thing that bothers me is that my front teeth are pretty flared forward and I have a deep bite.

These are the numbers from my ceph:

  • Upper incisors: 122°
  • Lower incisors: 115°
  • The orthodontist also mentioned a “severe dentoalveolar protrusion”
  • Upper jaw described as “contracted”
  • There’s supposedly a slight Class II tendency on one side, but clinically my bite looks fine to me.

Their plan is to extract four premolars and then use braces to pull everything back. They told me that without extractions they wouldn’t be able to retract the incisors properly or get a stable bite in the back.

My main doubt is pretty simple:

Does it actually make sense to remove four healthy premolars just to fix protrusion, especially if my molars and canines already look Class I?

Or should I get a second opinion before agreeing to something irreversible?

I’d really appreciate hearing from anyone who went through something similar or from orthodontists who can make sense of whether my numbers (122°/115°) actually justify extractions.

Thanks a lot to anyone who replies.
