Why do clinical notes say something about your personality?

This is not a medical question per se, but I think only doctors can answer it.

I recently had surgery and the clinical notes said “FlattenYourCardboard is a very pleasant xx-year-old woman with [condition].”

I don’t mind people saying that about me, but why is it in there? I don’t really see the medical relevance?
