What to do if there are no children?

For some reason, no characters in my game are having kids. I suspected it might be a mod issue, so I disabled all my mods, but now every character is over 40, so they can't have children naturally. Is my game just fucked? Or is there some way I can recover this? Does anyone know what happens to a country if there's no living characters and the ruler dies?
