Vyvanse 50 mg only lasts 4 h + constant tinnitus, clogged ears, light dizziness

Dosage question

Hi everyone,
45-year-old male, diagnosed with ADHD only this year.

Weeks 1–2: 30 mg Vyvanse
→ Worked fine but only lasted ~3 hours
→ Side effects were very mild (just dry mouth)
→ BUT the crash after those 3 hours was massive – totally drained and irritable

Week 3 onwards: increased to 50 mg
→ Now lasts maximum 4 hours (still followed by a horrible crash)
→ New side effects that are ruining my days:

  • Constant tinnitus/ringing in both ears (started around day 5 on 50 mg)
  • Ears feel clogged
  • Light Dizziness
  • Light-headedness every time I stand up quickly
  • Occasional vertigo

Psychiatrist wants to go up to 70 mg saying “side effects can get better with time or higher dose for a fast metabolizer”, but I’m terrified it will just make the tinnitus and dizziness worse while the duration stays ridiculously short.

  1. Anyone 40+ who had this exact pattern (tolerated 30 mg well → terrible ear + dizziness sides on 50–70 mg)?
  2. Did your tinnitus ever go away while staying on Vyvanse, or only after lowering the dose or switching meds?
  3. Anyone who switched to Concerta/Medikinet Retard, Adderall IR, or another stimulant and saw the tinnitus/clogged ears disappear?

When the medication is working I finally feel normal and productive for the first time in my life, but living 20 hours a day with ringing ears and dizziness is not an option at 45.

Thanks.
