CD Rip Settings help... How to set it back? To have No Track# in filename & Track# in Metadata

I was ripping CDs just fine, I altered the settings briefly & although I thought I'd set it back to exactly as before, when I rip CDs now, no track# is present in metadata & the track# is named at the start of the file.

Any tips to get this back as I had it would be very appreciated.
