Foot, calf hamstring tightness

I have DDD in s1 L5 and for the past 9 months I've been dealing with back pain. Recently had a lumbar ablation and epidural. 3 months ago I started getting nerve tingling in between my big toes. Over the past 3 months the tingling has been getting worse.

Has anyone else dealt with this and is surgery my only option?
