Sick Betta

I have a betta. I've had him for a little over two years. I think he has Ich and Popeye. I've tried my best to help him but a lot of the medicines and chemicals people recommend aren't really accessible where I live. His colour's starting to fade and Im so scared he's gonna die. I do weekly water changes and feed him regularly. I tried melafix but it wasn't working.

If I can't help him in the next few months, I'm going to try and rehome him somewhere he can be helped properly.

I'll take any advice, even where I can look for a new home. I love him and I'll do anything to make sure he doesn't die :(

He's in a 5gal tank with plants and some artifical decor. It has a sponge filter and a heater. Tank temp is 73°F. The tank is new this summer, he was sick in his old tank that's why I switched him. I've had him over 2yrs. I change the water weekly, i remove half with a gravel vacuum and refill with conditioned water. No tank mates. I feed him betta food pellets, 3 twice a day.
