Hello!

For the past 4+ months I've been having this RUQ pain. The pain is very cramping and sharp, not very consistent with any type of food. There is no tenderness to the touch. No pain when palpated. Pain mainly is right-center under ribcage but sometimes will be a little more toward the right side. My diet has basically just turned to bland crackers or rice and meal replacement shakes because I'm afraid to make the pain worse. I've lost like 20+ lbs since it started.

Lipase serum and liver profile labs done as of 9/15, lipase came back normal and the only out of range result for the liver profile was ALT/SGPT was 10 u/L. As of 8/15 Amylase was 23 u/L. Absolute Neutrophil count tested and that was slightly elevated at 7.76 10x3/ul and Segs slightly elevated at 78%. Everything else on Chem 7, HCG quantitative, lipase, liver, hemogram, platelet, differential seems normal.

CT, MRI, and Ultrasound appear are normal. Had Gastric emptying test that showed delayed emptying at 54% and had a HIDA scan that shows a 73% ejection rate w/ no blockage. Colonoscopy on 10/31 showed only mild inflammation from gerd, pathology clear.

Since it seems that gallbladder is basically cleared, I am at a complete loss of what this could be. My GI thinks it's IBS, second opinion GI thinks It could be the gastroparesis. I'm wondering if it could maybe be kidneys? I know the usual kidney pain would be in you back or sides but I'm thinking this because I have been having an issue of recurrent UTI's and when I get them my only symptom is abdominal pain. I had a UTI a few months ago, took keflex and symptoms went away but the infection did not, but nothing was done after the culture showed mixed flora. I'm wondering if maybe because nothing was done to fully get rid of the UTI, it went to my kidney and that's what this pain is?? I just tested positive for a UTI again and have been on macrobid and d-mannose for the past few days, but the culture came back as mixed flora again so idk if anything will be done just like last time.

Is there anything else that could explain these symptoms with all of these tests coming back normal?? It's really effecting my quality of life but I don't know what to do. I wanted to ask here so if anyone has any ideas I can advocate for myself and get further testing to figure out what's wrong...
