Best providers in chronic care for transgenders ?

Im interested to know any primary care (physicians or physicians assistants, etc, doesn't have to be a doctor) folks who are good with the intersection of trans bodies and chronic health conditions? I'm less in need of trans healthcare than chronic care currently; but I also want whoever I work with to be well educated and not a dick 🙃
