Grain effect vs low NR

When shooting in low light using a recipe with -4 NR and large strong grain what’s the difference between removing the grain effect or bringing the noise reduction back up. I understand one is removing natural grain and one is artificially superimposing grain but how does the actual image get impacted in terms of the look and quality?
