20 M ("afab") Urinary Retention AND Urinary Incontinence? Doctors Unhelpful

Hello, I'm a transsexual man, I've been taking testosterone (0.5mL/week) for about 2 years. I'm also on accutane I'm 5'8 and 170lbs. No sex reassignment surgery yet. I've asked my endocrinologist and a doctor at my university's clinic but they were both kind of dismissive which was ok at first but these issues are effecting my QOL more and more. I asked my endocrinologist because I'd heard that testosterone can affect pelvic floor stuff and thus bladder stuff but he said ahh no you have to talk to someone else sorry! I'm kinda poor I can't be making a million appointments yknow. Anyways, I constantly feel the urge to urinate and despite this can NEVER fully empty my bladder, it's uncomfortable. And lately, on-and-off but happening more this week, I'm having leakage throughout the day, I'll just like randomly feel wetness and usually assume it's sweat but no it's actually urine;-; I don't get how both of those are happening at the same time but any help would be much appreciated, thank you
