Mental health continues to deteriorate, with particular concerns about young people

The mental health of the Dutch population is deteriorating. This is according to researchers from the National Institute for Public Health and the Environment (RIVM) and the Trimbos Institute in the new Mental Health Monitor. Not only are more people struggling with mental health problems, but the number of people with a mental illness is also increasing.

"This is a truly worrying deterioration, which had already begun before the coronavirus pandemic," says Marith Volp, Director of Public Health and Healthcare at the RIVM (National Institute for Public Health and the Environment). "Mental health is under pressure."

Young people

The institutes find it difficult to pinpoint the source of the deterioration. They did, however, examine factors related to mental health. Insecurity is often cited. "Consider, for example, people who can't find housing."

Things are especially bad for young people. "We're really concerned about young people," says Volp. This specifically concerns the group under 25.

Other studies have also shown that young people's mental health is deteriorating. Girls, in particular , suffer significantly from conditions such as depression.

What are the consequences of the deterioration of our mental health?

Mental health problems are putting pressure on Dutch healthcare. A quarter of Dutch people over the age of 16 have already visited their general practitioner for mental health issues, and one in twenty has seen specialized mental health care.

"The disease burden for mental health problems is higher than for any other disease," says Volp. Disease burden is calculated by looking at the number of years of life lost due to a disease, plus the number of years lived with the disease.

The declining mental health also costs the Netherlands dearly financially. "That amounts to almost €500 in medical care costs per resident per year. Moreover, absenteeism due to mental health issues costs the Netherlands around €5 billion annually," says the RIVM director.

The suicide rate has remained stable in recent years, the researchers say.

On behalf of both institutes, Volp is appealing to the parties forming a coalition government in The Hague. "Mental health must be high on their agenda. In the Netherlands, we must focus heavily on prevention and early detection," she says.

"Prevention isn't just a responsibility for healthcare, but for society as a whole. This way, we also keep healthcare costs manageable. Moreover, we need long-term policies, which we haven't had in recent years."

Gideon Sterkenburg, director of Neon College for special secondary education

Schools in Enschede are already working on prevention and early detection. Gideon Sterkenburg is the principal of Neon College, a secondary school for special education. The school has a youth care worker who works closely with teachers.

"The collaboration between youth services and teachers means we're much more aware of each other. That works really well," says Sterkenburg. "Normally, there's no real contact between youth services and the school, but this way there is. The prevention also lies in the fact that we can learn from each other more quickly."

Having a youth care worker present at the school also makes it easier to quickly identify signs in students or to talk to parents about parenting and possible youth care needs. "You can much more quickly ask the care professional to join you if they're down the hall."

"It's also helpful that someone from the school can directly refer patients to other healthcare organizations. Normally, the school isn't involved in such processes."

Fear of failure

Sterkenburg cautions that calling on youth services for students experiencing difficulties isn't always the best option. "That way, you're telling the student that they absolutely need youth services."

The principal says that young people's problems don't always need to be immediately medicalized. "For example, maybe a student has test anxiety, or maybe they simply don't want to study for tests. By looking at things from different perspectives, you can already solve a lot."

Sometimes the solution to mental health problems doesn't lie with healthcare, says Sterkenburg. "A student's lack of sleep could simply be due to their phone use. So you have to address that. And don't underestimate the work of skilled social workers."
