Dusk of Oolacile hasn't spawned

I just killed the Hydra and Golden Golem and after it died she just didn't spawn. I've reloaded and checked for the sign and nothing. The golem was on a weird bit of geometry, could she have fallen through the map? I used pyromancy would that be the issue?

As far as I can tell I have no way of actually meeting her which is extremely frustrating, is there anything I can do? Save editing or something? Thanks for any help folks
