Rescued sick feral 3 months ago - still will not move from closet

I rescued this little thing about 3 months ago, I found it living between a 6inch gap between 2 buildings. I was monitoring it over a month to ensure there was no colony and to see if it would get better. It continued to get worse, with incredibly matted fur and not moving much.

Live trapped it and made a safe space in my large closet. while it’s doing so much better and has almost fully de-matted itself, it will NOT let me get near it yet. It has been trapped and released before (ear clipped) could that be why it’s so hard to socialize?

I want to get this baby to a vet for a checkup and to have fleas removed. I would love to keep it, but I feel like I’m holding it captive 😭
