Title transfer help (CA)

Long story short, bought a car for my ex wife but under my name, after divorce i let her have it, signed over the title and gave it to her. However i kept getting registration notices in the mail, and after checking online I see that the car is still registered to my name. Does this mean she never finished the process on her end? And if so what can i do to get my name off the title?
