Am I doing something wrong to not get the Survive Boon from Echo?

I've been trying to finish the prophecy of getting one of every boon from Echo, but for some reason she never offers Survive Survive Survive. I have been running death defiances, and even am reaching her making sure at least one of my DD's are depleted. Despite doing this for at least 10+ runs in a row she still never offers the boon. She only has 7 boons to offer, so it should be a 3/7 chance each time? Am I just getting insanely unlucky? Wondering if anyone else has ran into this problem.
