Multiple Monitors - updates keep screwing things up

I've run three monitors since about 2007 - currently on Windows 11 using an NVidia gfx card. Every now and then, I start Windows and find the taskbar has moved to the lefthand monitor and apps no longer maximise on the monitor they're on - they maximise across all three. I can still game with three monitors, but how I had it was three monitor gaming and Windows maximising on one monitor with the taskbar in the centre.

Every time this happens I fiddle around for ages and something puts it back to how it was. I'm then fine for a month or two and it happens again.

Does anyone know a) what's going wrong, b) how to prevent it, and c) how to revert it back without spending an hour fiddling around each time? I'm getting fed up with it. Just as it's fixed, a few weeks passes and then boom.
