After birth support for mother's in different states

My sister just had her first kid and is really struggling. We live in different states and I remember have a nurse visit my house to check on me and see how I was doing free of charge. I also remember having a lot of support from medical professionals focusing on my care in addition to the baby. From talking to her she gets one check up in 6 weeks and other than that has not had much support.

I'm curious what others have experienced in care for the new mother.
