I almost hit a pedestrian today, how do I fix this?

USA driver here

I was backing out of my driveway and into the far lane, and suddenly I see an elderly man standing right by my back passenger door, head bent down as if trying to see what I look like. He was guaranteed to be within arms length of my car. And of course I was so surprised by him suddenly being there I panicked and drove off before even realizing that I probably almost ran him over!!!

The only way he could’ve been standing there so close after I’ve backed onto the road is if I failed to look everywhere behind me before pulling out, which tells me this isn’t the first time I’ve been this negligent. This is completely unacceptable behavior, and I no longer trust myself to be a responsible driver.

Also a few weeks ago I nearly hit an entire couple who were trying to cross the parking lot I was driving in. Didn’t see them until I braked and suddenly they came into view, as they were concealed behind the A-pillar because apparently this is a blind spot that I was too stupid to notice as a potential hazard. And they suddenly stopped and kinda jumped as well, so obviously I nearly hit them.

So that’s 2 near misses in as many months. Unacceptable. This is becoming a pattern. What do I need to do to fix this issue? Who should I report this to? I can’t just sit here and be a danger to people, and I don’t think I’m a good driver anymore, and there need to be consequences for this behavior.

Thank you for any info
