Hazard OTP VOD Review Request (D2-M4)

I only play Hazard. I'II have games like this where it's either a JQ or Zarya against me and there's seemingly nothing I can do to turn the tides, they seem to walk through my team regardless if I try to frontline or dive. This game is very representative. Other than some of the more blatant feeds, where am I going wrong? Looking to incorporate some fundamental changes to how I play Hazard in this kind of matchup.
