Forgetting she can't walk. Falling.

What more can I do? My mom broke her femur on Christmas. She has been a bit behind the curve with rehab, and has had a painful start. Problem is, she has fallen 6 times since her discharge. Three times today. She forgets she's not able to get up un assisted, and down she goes. I plastered her room with signs that say
"NAME," STOP! you are a FALL RISK ! DO NOT GET UP WITHOUT HELP (you broke your leg)

She was so mobile before this, that she just isn't used to needing help. I know she sundowns, so naturally, even the visual cues are not going to be as effective during those hours. But ...any suggestions? Yes, she has bed & chair alarms, but those are only effective for getting help after the fact.
