Migraine but mostly constant pain under scalp 24/7 - very close to allodynia

Hello everyone,

I would really appreciate some insight or shared experiences.

I've got Migraine for year and half

I have completed the full course of Topamax (topiramate) and Tofisopam as prescribed by my neurologist, but unfortunately I did not notice significant improvement.

For the past ~2 weeks, I have been taking supplements including:

   •   Magnesium glycinate

   •   CoQ10

   •   Vitamin B2

   •   Vitamin B12

So far, I also have not noticed any clear benefit from these.

The most noticeable improvement has come from lifestyle changes, especially:

   •   Reducing my computer work intensity

   •   Avoiding carbohydrates

However, my main symptom persists:

I have a constant, dull, aching pain on a specific area of my scalp, present almost 24/7 except when sleeping.

It does not come with typical migraine symptoms such as:

   •   Light sensitivity

   •   Sound sensitivity

   •   Eye pain or orbital pain

What I experience instead:

   •   Surrounding scalp areas are painful only when touched (similar to scalp allodynia)

   •   But one focal area sends continuous pain signals even without touch

   •   The pain becomes more noticeable toward the end of the day

   •   When I pull the skin above my eyebrow and close my eye, the pain signal temporarily disappears

Because I currently have gastritis, I avoid caffeine so I cannot experiment it at this time.

My concern is whether:

   •   I should return to my neurologist for re-evaluation, or

   •   Continue being patient with supplements and workload adjustment

I am particularly worried that if this condition worsens or if more symptoms appear, it may become irreversible. I understand that migraine is generally considered a chronic condition that can be managed but not cured, and that concerns me.

Has anyone experienced a similar scalp-dominant migraine or nerve-related pain pattern?

Did changing preventive medication, treatment strategy, or diagnosis help?

Thank you very much for reading.
