How can I book an appointment in CMC vellore?

Hi I'm from Jharkhand and I'm planning to visit CMC vellore for my shoulder's treatment so how can I book the appointment for it and also which OPD type should I prefer General or Private? as it is listed on their website.

Please help me with this and if you know any place for accomodation around hospital then let me know also about it.
