Thoughts Please
I have a patient who I have been seeing now well over 4 months for posterior tibialis tendonitis and as of recently low back pain (which over the past 4 months I have never noticed to be an issue). She is not compliant with her HEP and consistently shows up late to appointments. She lets me know her doctor wants her to continue 3x/week for 6 weeks (we have been doing 2x/week) I let her know that based on her presentation we are doing 2x, well she tells me after all this time in PT she is not doing better. So I let her know that we may need to refer back to PCP if after all this time she feels no difference with PT (mind you not HEP compliant, not very active), so now she tells me that she feels she has improved and PT is helping.....
Not sure how to feel, part of me is frustrated because I dont know how discharge her but also not sure what subjective improvements we have made. I almost feel as though she comes in to exercise, but doesn't really understand the concept of PT. I have spent a lot of time doing pt education..
For context, since IE she had very minimal ROM/ strength limitation, her main impairment is gait mechanics (due to pain) and intrinsic foot strength. Which has shown little to no change over the course of her POC. (also little off topic mildly infuriating every time I do soft tissue she literally moans, like uncomfortable for the rest of the clinic to the point that I cant do soft tissue its just very uncomfortable.)
Maybe I'm just ranting but tell me what you think.
