Low-dose Rubifen (5mg IR 2x/ day) has been transformative for my adult ADHD - my experience so far

Questions/Advice

Hey everyone,

Recently diagnosed with adult ADHD (mostly inattentive with executive function and motivation struggles). Symptom lists clicked slowly the more I felt the drag recently after a few months of dealing with grief from my mom's death: constant mental noise, tasks a slog with only relief when done (not reward), easy loss of focus, constant intrusive thoughts.

Doc prescribed Rubifen 5mg IR (methylphenidate hydrochloride same as Ritalin generic) start 5mg once then 2x daily if needed. First 5mg dose: mind quieter than I ever knew possible, focus much easier, tasks suddenly "manageable" without the fight. Genuinely transformative. Life flows smoother, brain noise gone which I didn't even know what non-noise felt like or that I had noise at all, motivation is natural. Pre-med days feel like another reality.

By day 2 it was clear I needed a second dose for full coverage so I added it and it's been working much better. No anxiety increase (big worry), no jitteriness, no emotional flatness. Side effects minimal: libido unchanged or slightly better from less distraction.

Each dose lasts ~5-6 hours.

Mild energy crash/fatigue late afternoon/evening (post dinner) with subtle creep back (more distractibility or effort needed) but nothing like untreated. No dramatic rebound; one mild irritable evening (likely low blood sugar) and one exhausted day after training (possibly same reason).

Doc said I could go 10mg 2x (already prescribed) but I'm happy with 5mg peak now. Unimaginable improvement already. Today I'm experimenting with the 3rd dose of 5mg, took it at 4PM.

Anyone have experience with deciding between two bigger doses vs 3 smaller ones?

Hesitant to increase at this stage.

Thanks for reading! This sub was a big help in getting things kicked off :)
